Exploiting his medical history, the male victim (18 years old, in Ba Vi, Hanoi) worked as a car repairman at a garage in Cau Giay district. After pumping, the victim sat on the tire to reinstall the tire valve, but unfortunately the tire exploded. The explosion caused the victim to be hit hard in the chest wall by the tire wall and fainted on the spot.

Nguyen Hoang Nam – Department of Adult Cardiovascular Surgery said that the victim was admitted to the hospital with severe chest pain , several bruises on the chest wall between the sternum and breathing oxygen maskā€¦ Previously, The victim was given first aid at another hospital. However, after the necessary x-ray, doctors suspected that the victim had damage to the aorta that needed emergency treatment, so they immediately transferred him to the Cardiovascular Center, E Hospital.

Doctors immediately gave the victim the necessary tests such as echocardiography and chest wall CT with contrast injection to accurately assess the damage caused by the victim’s chest injury.

The results showed that the victim suffered a ruptured aortic isthmus after trauma. Doctors conduct consultations. Dr. Nguyen Cong Huu – Director of Hospital E and the doctors determined that this was an emergency case that needed urgent surgery for the victim. Doctors decided to surgically place an endovascular stent-graft with a sheath from the femoral artery up to cover the aortic isthmus tear, with a window opening for the victim’s left subclavian artery. After intervention, the patient’s overall hemodynamic status and vital signs are controlled.

MSc. Nguyen Hoang Nam shared that aortic isthmus rupture is rare and very dangerous. Can occur in accidents with a very large impact force on the chest, such as a car accident or a high fall. The aortic isthmus is the most vulnerable place because the aortic isthmus is the junction between the fixed part, the aortic arch, and the mobile part, the descending aorta.

If a person with a ruptured aortic isthmus is not treated promptly, the risk of death in the first 6 hours is 24%, and up to 50% in the first 24 hours for severe injuries.

Thoracic aortic rupture is a rare and dangerous disease, but it is possible to accurately diagnose and detect damage early to plan treatment, intervention, surgery, and diagnostic imaging techniques such as X-rays, CT scan with contrast, transesophageal echocardiography..

For aortic injuries, conservative medical treatment can be applied for mild cases (type I, II) and surgery for severe cases (type III, IV). Surgery to open the chest, run an extracorporeal circulator with the purpose of stopping the patient’s heart, then cut and replace the circuit in the chest. This is a very serious surgery, with a high mortality rate accompanied by spinal paralysis and a long postoperative recovery period.
